The first of the two smaller back teeth (premolars) on the lower right, just behind the canine.
- In front: #27 lower right canine
- Behind: #29 lower right second premolar
- Same tooth on the other side: #21 lower left first premolar
- Bites against: #5 upper right first premolar
It replaces the lower right baby first molar (S), which falls out at 9–11 years.
Usually one root (1–3) and usually one canal (1–3).

This tooth is #28 in the US and 44 in the international (FDI) system, where the 4 means the lower right quarter and the 4 is its place counting from the front.
In the international system, 28 is the upper left wisdom tooth (#16).
