The second of the two premolars on the upper right, just in front of the first molar.
- In front: #5 upper right first premolar
- Behind: #3 upper right first molar
- Same tooth on the other side: #13 upper left second premolar
- Bites against: #29 lower right second premolar
It replaces the upper right baby second molar (A), which falls out at 10–12 years.
Usually one root (1–3) and usually one canal (1–3).

This tooth is #4 in the US and 15 in the international (FDI) system, where the 1 means the upper right quarter and the 5 is its place counting from the front.
There's no international tooth 4, because those numbers always have two digits.
And in the US, #15 is the upper left second molar.
